Publication & Policy Statement: Footnotes (USP 941-060) is published monthly except July & August, by Square and Folk Dance Federation of Washington, a Non-Profit Corporation. Subscription rate is $15.00 per year. Canadians pay $20.00 (US) per year. New Subscriptions start with the next issue IF we receive this completed form on or before the 10th of the month. In the event circumstances are such that FOOTNOTES ceases to be printed, there will be no reimbursement of subscription payments. The editors reserve the right to condense, omit or rewrite all or any part of material received for publication and exercise their discretion in the acceptance of any material. The right to reprint articles or art work included in FOOTNOTES is given provided credit is given to FOOTNOTES.
